Please bring the following documents for enrollment in State Funded Programs:

  1. Proof of income (pay stubs for the last 2 months)

  2. Proof of residency

  3. Birth certificates for each child under 18 living in the home

  4. Immunization card with current TB test

  5. Income guidelines, eligibility based on need & income
